Summary
Amiodarone prolongs the QT interval, and thiazide-induced hypokalemia or hypomagnesemia increases the risk of torsades de pointes. Amiodarone also inhibits CYP3A4 and may modestly raise amlodipine levels, adding to hypotension or bradycardia.
What this interaction means in plain words
Amiodarone and Amlodipine-hydrochlorothiazide-valsartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amiodarone belongs to Cardiovascular; Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular.
Why it happens
Thiazide-induced electrolyte depletion potentiates amiodarone QT prolongation; amiodarone also weakly inhibits CYP3A4 metabolism of amlodipine.
What to do
Monitor potassium and magnesium and correct deficiencies, and obtain periodic ECGs. Watch for hypotension and bradycardia.
What to ask your pharmacist
