Summary

Duvelisib is a CYP3A4 inhibitor and can raise amiodarone exposure, while amiodarone may increase duvelisib exposure through P-glycoprotein inhibition and as a CYP3A4 substrate interaction. Higher amiodarone levels increase the risk of bradycardia, QT prolongation and other dose-related toxicity.

What this interaction means in plain words

Amiodarone and Duvelisib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Duvelisib belongs to Oncology.

Evidence level: Low

Why it happens

Duvelisib inhibits CYP3A4 (reducing amiodarone metabolism), and amiodarone inhibits P-glycoprotein efflux.

What to do

Monitor ECG and heart rate for signs of amiodarone excess, and watch for duvelisib adverse effects such as infection, diarrhea and rash. Reduce the amiodarone dose if toxicity appears.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
