Summary
Amiodarone prolongs the QT interval and thiazide-induced hypokalemia or hypomagnesemia increases the risk of torsades de pointes. The legacy suggestion of altered thiazide clearance is of little clinical importance compared with this electrolyte effect.
What this interaction means in plain words
Amiodarone and Enalapril-hydrochlorothiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amiodarone belongs to Cardiovascular; Enalapril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Thiazide-induced hypokalemia and hypomagnesemia potentiate amiodarone-related QT prolongation.
What to do
Monitor potassium and magnesium and correct deficits. Obtain an ECG if other QT-prolonging drugs are present or if electrolytes are abnormal.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
