Summary

Amiodarone markedly prolongs the QT interval and salbutamol can cause tachycardia, hypokalemia and arrhythmias, particularly at high doses. Together they may raise the risk of torsades de pointes or other ventricular arrhythmias.

What this interaction means in plain words

Amiodarone and Ipratropium-salbutamol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Ipratropium-salbutamol belongs to Respiratory.

Evidence level: Moderate

Why it happens

Additive proarrhythmic effects: amiodarone QT prolongation plus beta-2 agonist induced hypokalemia and tachycardia.

What to do

Monitor ECG and serum potassium and magnesium, and correct deficits. Use the lowest effective salbutamol dose and avoid frequent high-dose nebulization.

What to ask your pharmacist

  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
