Overview

Severity: Major · Evidence level: High

Additive QT prolongation increasing risk of torsades de pointes and ventricular arrhythmias when a fluoroquinolone is used with a class III antiarrhythmic.

Mechanism of action

Pharmacodynamic: both prolong cardiac repolarization (QTc) via IKr blockade, leading to additive QT prolongation.

Clinical management

Avoid combination if possible; use a non–QT-prolonging antibiotic. If necessary: obtain baseline and follow-up ECGs, correct K/Mg, avoid other QT-prolongers, monitor for syncope/palpitations; adjust levofloxacin for renal function.
