Summary

Amiodarone may raise sparsentan exposure through CYP3A4 inhibition, which could increase hypotension, edema, hyperkalemia and renal effects. Sparsentan, a weak CYP2C19 inducer, could in theory lower exposure to CYP2C19-metabolized drugs, though this is of uncertain relevance for amiodarone.

What this interaction means in plain words

Amiodarone and Sparsentan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Sparsentan belongs to Genitourinary.

Evidence level: Low

Why it happens

Amiodarone inhibits CYP3A4, which metabolizes sparsentan; sparsentan induces CYP2C19.

What to do

No sparsentan dose change is generally required, but monitor blood pressure, serum potassium, edema and renal function. Also watch for loss of amiodarone effect.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
