For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Terbinafine inhibits CYP2D6 and may modestly affect other metabolic pathways. Amiodarone has a very long half-life and both drugs are associated with hepatotoxicity and, rarely, QT effects. Documented pharmacokinetic interaction is limited, and the legacy mechanism via CYP2C9 raising terbinafine is poorly supported.

What this interaction means in plain words

Amiodarone and Terbinafine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amiodarone belongs to Cardiovascular; Terbinafine belongs to Anti Fungals.

Evidence level: Low

Why it happens

Possible inhibition of terbinafine clearance by amiodarone and additive hepatotoxicity; the mechanism is not firmly established.

What to do

Monitor liver function tests and ECG during combined use. Consider an alternative antifungal if hepatic or cardiac abnormalities appear.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
