Summary

Carbidopa is given with levodopa, and levodopa-carbidopa can cause orthostatic hypotension. Combined with amlodipine, blood pressure lowering may be additive, especially when dopaminergic therapy is started or increased. Carbidopa alone has little hemodynamic effect.

What this interaction means in plain words

Amlodipine and Carbidopa can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine belongs to Cardiovascular; Carbidopa belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive hypotensive effects of levodopa-related dopaminergic therapy and calcium channel blocker vasodilation.

What to do

Check blood pressure, including standing readings, when levodopa-carbidopa is started or titrated in patients on amlodipine. Reduce the amlodipine dose if symptomatic hypotension occurs.
