Summary

Cyclosporine raises atorvastatin exposure markedly through OATP1B1 and CYP3A4 inhibition, increasing the risk of myopathy and rhabdomyolysis, and may also raise amlodipine levels. Amlodipine can modestly increase cyclosporine trough concentrations.

What this interaction means in plain words

Amlodipine-atorvastatin and Cyclosporine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Evidence level: High

Why it happens

Cyclosporine inhibits CYP3A4 and OATP1B1, and amlodipine may modestly raise cyclosporine levels through CYP3A4/P-gp effects.

What to do

Avoid atorvastatin with cyclosporine, as labeling advises, and use an alternative statin at a restricted dose; monitor cyclosporine trough levels and blood pressure if amlodipine is used.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
