Summary

Apalutamide is a strong CYP3A4 inducer and can substantially lower exposure to amlodipine and atorvastatin, both CYP3A4 substrates. This may lead to loss of blood pressure control and reduced lipid-lowering efficacy. The induction effect develops over days to weeks and persists after apalutamide is stopped.

What this interaction means in plain words

Amlodipine-atorvastatin and Apalutamide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-atorvastatin belongs to Cardiovascular; Apalutamide belongs to Endocrine.

Evidence level: Moderate

Why it happens

Induction of CYP3A4 (and related transporters) by apalutamide increases the metabolism of amlodipine and atorvastatin.

What to do

Monitor blood pressure and LDL cholesterol, and increase doses of amlodipine or atorvastatin if needed, or switch to a less CYP3A4-dependent agent. Reassess doses after apalutamide is discontinued.
