Summary
Atazanavir inhibits CYP3A4 and raises amlodipine exposure, and boosted regimens with ritonavir or cobicistat also raise atorvastatin exposure. This increases the risk of hypotension, edema and statin myopathy.
What this interaction means in plain words
Amlodipine-atorvastatin and Atazanavir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-atorvastatin belongs to Cardiovascular; Atazanavir belongs to Anti Virals.
Why it happens
Inhibition of CYP3A4 (and OATP with boosters) reduces clearance of amlodipine and atorvastatin.
What to do
Use a reduced amlodipine dose, with separate agents, and monitor blood pressure and ECG for PR prolongation. Keep atorvastatin at the lowest effective dose and watch for muscle symptoms.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
