Summary

Nirmatrelvir is given with ritonavir, which strongly inhibits CYP3A4 and increases exposure to atorvastatin and amlodipine. This raises the risk of hypotension, edema and statin-related myopathy.

What this interaction means in plain words

Amlodipine-atorvastatin and Nirmatrelvir can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-atorvastatin belongs to Cardiovascular; Nirmatrelvir belongs to Anti Virals.

Evidence level: Moderate

Why it happens

Ritonavir-boosted nirmatrelvir inhibits CYP3A4, decreasing the clearance of amlodipine and atorvastatin.

What to do

Consider holding the fixed-dose product during the short antiviral course, or substitute separate agents with dose reduction of amlodipine. Monitor blood pressure and muscle symptoms during and for several days after treatment.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
