Summary

NSAIDs such as celecoxib can blunt the antihypertensive effect of ARBs and thiazides and, in combination with them, increase the risk of acute kidney injury, particularly in elderly or volume-depleted patients. Hyperkalemia may also occur because of reduced renal prostaglandin-mediated effects on potassium handling.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Celecoxib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Celecoxib belongs to Musculoskeletal.

Evidence level: High

Why it happens

Inhibition of renal prostaglandin synthesis causes sodium and water retention and lowers renal perfusion, opposing the antihypertensive agents and adding to renal hemodynamic stress.

What to do

Use the lowest effective celecoxib dose for the shortest period and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, especially after starting therapy.
