Summary
Diclofenac can reduce the antihypertensive efficacy of valsartan and hydrochlorothiazide. The combination of an NSAID, ARB and diuretic is associated with acute kidney injury, particularly in elderly or volume-depleted patients, and may also cause hyperkalemia.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Diclofenac can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Diclofenac belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandins reduces afferent arteriolar dilation and promotes salt retention, while ARB and diuretic reduce glomerular perfusion pressure and volume.
What to do
Avoid regular NSAID use where possible and prefer paracetamol or topical options. If needed, use the lowest dose briefly, maintain hydration and monitor blood pressure, creatinine and potassium.
