Summary
NSAIDs such as etodolac blunt the antihypertensive effect of valsartan and hydrochlorothiazide through sodium and water retention. In combination with an ARB and a diuretic, NSAIDs can also cause acute kidney injury, particularly in older or volume-depleted patients, and may raise potassium.
What this interaction means in plain words
Amlodipine-hydrochlorothiazide-valsartan and Etodolac can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Etodolac belongs to Musculoskeletal.
Why it happens
Inhibition of renal prostaglandin synthesis reduces renal blood flow and promotes sodium retention, opposing antihypertensive effects and impairing glomerular filtration when angiotensin II action is blocked.
What to do
Use the lowest effective NSAID dose for the shortest time, maintain hydration, and monitor blood pressure, creatinine and potassium. Consider paracetamol or another alternative if appropriate.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
