Summary

Quinidine is a basic drug that is secreted by renal tubular cation transport and may compete with hydrochlorothiazide for tubular secretion. This could modestly raise hydrochlorothiazide exposure, although the clinical effect is generally small. Thiazide-induced hypokalemia or hypomagnesemia also raises the risk of quinidine-related QT prolongation.

What this interaction means in plain words

Amlodipine-hydrochlorothiazide-valsartan and Quinidine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-hydrochlorothiazide-valsartan belongs to Cardiovascular; Quinidine belongs to Anti Parasitic.

Evidence level: Low

Why it happens

Competition for renal tubular secretion of hydrochlorothiazide, plus thiazide-induced electrolyte disturbances that augment quinidine proarrhythmic effects.

What to do

Monitor serum potassium and magnesium and correct deficiencies. Obtain an ECG if QT-related symptoms or other QT-prolonging drugs are present.
