Summary

Meloxicam can attenuate the antihypertensive effect of olmesartan and hydrochlorothiazide, and the combination of an NSAID, an angiotensin receptor blocker and a thiazide diuretic (the so-called triple whammy) increases the risk of acute kidney injury and hyperkalemia. Older and volume-depleted patients are at highest risk.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Meloxicam can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Meloxicam belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAID inhibition of renal prostaglandins causes sodium retention and afferent arteriolar constriction, which combined with angiotensin II blockade and diuresis impairs glomerular filtration.

What to do

Avoid or minimize NSAID use and ensure adequate hydration. Monitor blood pressure, serum creatinine and potassium, especially in the first weeks.
