Summary
Nilotinib inhibits CYP3A4 and may raise amlodipine concentrations, increasing the risk of hypotension and edema. Nilotinib also prolongs the QT interval, and thiazide-induced hypokalemia or hypomagnesemia could add to that risk.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Nilotinib can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Nilotinib belongs to Oncology.
Why it happens
Inhibition of CYP3A4 by nilotinib reduces amlodipine clearance, and diuretic-related electrolyte loss adds to QT risk.
What to do
Monitor blood pressure and edema and reduce the amlodipine dose if needed. Keep potassium and magnesium within normal ranges and monitor the ECG as recommended for nilotinib.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
