Summary
Both salmeterol and hydrochlorothiazide can lower serum potassium, and the effect may be additive, particularly at high beta-agonist doses. Hypokalemia can promote arrhythmias, especially in patients with heart disease or hypoxia.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Salmeterol can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Salmeterol belongs to Respiratory.
Why it happens
Beta-2 agonism drives potassium into cells while thiazides increase renal potassium loss, producing additive hypokalemia.
What to do
Monitor serum potassium in patients on regular or high-dose salmeterol and replace potassium as required. The olmesartan component may partially offset losses.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
