Summary

NSAIDs such as tolmetin can blunt the blood pressure lowering effect of olmesartan and hydrochlorothiazide. In combination with a thiazide and an ARB, they raise the risk of acute kidney injury, particularly in elderly or volume-depleted patients, and may also promote hyperkalemia.

What this interaction means in plain words

Amlodipine-olmesartan-hydrochlorothiazide and Tolmetin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Tolmetin belongs to Musculoskeletal.

Evidence level: High

Why it happens

NSAIDs inhibit synthesis of vasodilatory renal prostaglandins, reducing renal perfusion and sodium excretion and opposing the antihypertensive and natriuretic effects of ARBs and diuretics.

What to do

Monitor blood pressure, serum creatinine and potassium when starting or stopping tolmetin, and ensure adequate hydration. Use the lowest effective NSAID dose for the shortest time, or choose an alternative analgesic such as paracetamol.
