Summary
Tolvaptan is a vasopressin antagonist that can produce marked water loss and sodium shifts, and it may modestly raise potassium. Combined with olmesartan (potassium retaining) and hydrochlorothiazide (potassium wasting) the net effect on potassium is unpredictable, and added volume depletion may cause hypotension and renal impairment.
What this interaction means in plain words
Amlodipine-olmesartan-hydrochlorothiazide and Tolvaptan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-olmesartan-hydrochlorothiazide belongs to Cardiovascular; Tolvaptan belongs to Endocrine.
Why it happens
Additive pharmacodynamic effects on fluid, electrolyte and potassium balance from aquaretic, thiazide and angiotensin receptor blocker actions.
What to do
Monitor serum potassium, sodium, creatinine and volume status regularly, especially after initiation. Adjust fluids and antihypertensive doses according to results.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
