For healthcare professionals — reference only. This page is a brief summary, not medical advice and not a substitute for official labeling or clinical judgement. Always verify before prescribing or dispensing.

Summary

Sacubitril/valsartan contains an angiotensin receptor blocker, and combining it with an ACE inhibitor such as perindopril produces excessive renin-angiotensin system suppression. This raises the risk of hypotension, hyperkalemia and renal impairment. In addition, concurrent ACE inhibitor use with sacubitril carries a risk of angioedema, and the combination is contraindicated.

What this interaction means in plain words

Amlodipine-perindopril and Sacubitril / valsartan are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.

Evidence level: High

Why it happens

Additive RAAS blockade, and neprilysin inhibition combined with ACE inhibition both impair bradykinin breakdown, raising angioedema risk.

What to do

Do not co-administer; stop perindopril at least 36 hours before starting sacubitril/valsartan. Choose a single agent acting on the RAAS.
