Summary

Loop diuretics such as ethacrynic acid add to the antihypertensive effect of valsartan and amlodipine and can cause volume depletion, hypotension and renal function decline. Potassium loss from the diuretic is partly offset by the ARB.

What this interaction means in plain words

Amlodipine-valsartan and Ethacrynic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Amlodipine-valsartan belongs to Cardiovascular; Ethacrynic acid belongs to Cardiovascular.

Evidence level: Moderate

Why it happens

Additive blood pressure lowering through diuresis and RAAS blockade, with first-dose hypotension risk in volume-depleted patients.

What to do

Monitor blood pressure, volume status, electrolytes and creatinine when starting or adjusting therapy, and consider reducing the diuretic dose before starting the ARB.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
