Summary
Mefenamic acid can reduce the blood pressure lowering effect of valsartan and, in combination with an angiotensin receptor blocker, increases the risk of acute kidney injury and hyperkalemia. The risk is greater in elderly, volume-depleted or renally impaired patients.
What this interaction means in plain words
Amlodipine-valsartan and Mefenamic acid can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-valsartan belongs to Cardiovascular; Mefenamic acid belongs to Musculoskeletal.
Why it happens
NSAID inhibition of renal prostaglandin synthesis reduces renal perfusion and potassium excretion, opposing angiotensin receptor blockade.
What to do
Use the shortest possible NSAID course at the lowest dose with adequate hydration. Monitor blood pressure, creatinine and potassium.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
