Summary
Piroxicam can reduce the blood pressure lowering effect of valsartan by suppressing renal prostaglandins and promoting sodium and water retention. In older or volume-depleted patients it may cause acute kidney injury and hyperkalemia.
What this interaction means in plain words
Amlodipine-valsartan and Piroxicam can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Amlodipine-valsartan belongs to Cardiovascular; Piroxicam belongs to Musculoskeletal.
Why it happens
NSAID inhibition of prostaglandin-mediated renal vasodilation combined with angiotensin receptor blockade lowers glomerular filtration and antagonizes antihypertensive effect.
What to do
Use the lowest effective NSAID dose for the shortest time and maintain hydration. Monitor blood pressure, creatinine and potassium; consider paracetamol as an alternative.
What to ask your pharmacist
