Summary
Amobarbital can induce CYP1A2 and CYP3A4 and reduce clozapine concentrations, risking relapse. Both drugs cause sedation, and the combination of CNS depressants with clozapine raises concern for respiratory depression and hypotension.
What this interaction means in plain words
Amobarbital and Clozapine can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Amobarbital belongs to Central Nervous System; Clozapine belongs to Central Nervous System.
Why it happens
Barbiturate induction of CYP1A2 and CYP3A4 lowers clozapine levels, with additive CNS depression.
What to do
Monitor psychiatric status, sedation and respiratory function, and consider clozapine plasma level monitoring with dose adjustment. Re-titrate carefully if amobarbital is stopped to avoid toxicity.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
