Summary

Tricyclic antidepressants block norepinephrine reuptake, which can potentiate the pressor and arrhythmogenic effects of direct sympathomimetics and blunt the effect of indirect ones. Dopamine acts partly indirectly, so the response can be unpredictable, with exaggerated hypertension or arrhythmias possible.

What this interaction means in plain words

Amoxapine and Dopamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Moderate

Why it happens

Inhibition of neuronal norepinephrine reuptake and uptake of indirect sympathomimetics by amoxapine alters the response to dopamine.

What to do

Start dopamine at a low dose and titrate carefully with continuous blood pressure and ECG monitoring in patients on amoxapine.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
