Summary
Modafinil may inhibit CYP2C19, which can raise concentrations of some tricyclic antidepressants in poor metabolizers. Amoxapine is a tricyclic-type drug, so toxicity such as sedation, anticholinergic effects or arrhythmia is theoretically possible. The pharmacodynamic effects partly oppose each other.
What this interaction means in plain words
Amoxapine and Modafinil can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Possible CYP2C19 inhibition by modafinil raising tricyclic levels, plus additive cardiovascular effects.
What to do
Monitor for tricyclic adverse effects and heart rate, and reduce the amoxapine dose if toxicity appears. Consider an ECG in patients with cardiac risk.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
