Both drugs can prolong the QT interval. Doxepin, a tricyclic antidepressant, also has anticholinergic and arrhythmogenic effects, so combined use may raise the risk of ventricular arrhythmia, especially at higher doses.
What this interaction means in plain words
Anagrelide and Doxepin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Anagrelide belongs to Cardiovascular; Doxepin belongs to Dermatology.
Why it happens
Additive QT prolongation and cardiac conduction effects.
What to do
Obtain a baseline ECG and monitor the QTc, potassium and magnesium. Use the lowest doxepin dose, or choose an alternative in patients with cardiac risk.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
