Summary
Apalutamide is a strong CYP3A inducer and can markedly lower doravirine plasma concentrations. This may lead to loss of virologic suppression and selection of HIV resistance.
What this interaction means in plain words
Apalutamide and Doravirine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Apalutamide belongs to Endocrine; Doravirine belongs to Anti Virals.
Why it happens
Apalutamide induces CYP3A4, increasing the metabolism of doravirine, which is a CYP3A substrate.
What to do
Do not coadminister; choose an alternative antiandrogen or antiretroviral regimen without dependence on CYP3A exposure. Allow time for enzyme induction to wane after stopping apalutamide before relying on doravirine.
What to ask your pharmacist
- Is there an alternative to one of these two medicines for me?
- If both are essential, who is monitoring me and how often?
- Which symptoms mean I should stop and seek help straight away?
