Summary
Apalutamide is a strong inducer of CYP3A4 and also induces other enzymes, which can lower exposure to drugs cleared by these pathways. Venlafaxine is mainly metabolised by CYP2D6 with a minor CYP3A4 contribution, so the effect is probably modest, but loss of antidepressant effect is possible.
What this interaction means in plain words
Apalutamide and Venlafaxine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Apalutamide belongs to Endocrine; Venlafaxine belongs to Central Nervous System.
Why it happens
Apalutamide induces CYP3A4 (and other metabolic enzymes), potentially increasing venlafaxine clearance via its minor pathway.
What to do
Monitor for reduced antidepressant response and discontinuation-like symptoms when apalutamide is started. Increase the venlafaxine dose based on clinical response if needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
