Summary
Sotalol blocks beta-2 receptors and can reduce arformoterol's bronchodilator effect or provoke bronchospasm. Arformoterol may also lower potassium and prolong QT, increasing torsades risk with sotalol.
What this interaction means in plain words
Arformoterol and Sotalol can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Arformoterol belongs to Respiratory; Sotalol belongs to Cardiovascular.
Why it happens
Beta-2 antagonism opposes the agonist, while beta-agonist induced hypokalemia and QT effects add to sotalol's QT prolongation.
What to do
Avoid the combination if possible and choose an alternative to sotalol in patients with obstructive lung disease; if used together, monitor potassium, QTc and respiratory function.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
