Summary
Argatroban, a direct thrombin inhibitor, raises the INR on its own, so INR readings during combined therapy overestimate warfarin's effect. This can lead to premature argatroban discontinuation or incorrect warfarin dosing, with risk of bleeding or inadequate anticoagulation.
What this interaction means in plain words
Argatroban and Warfarin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Argatroban belongs to Cardiovascular; Warfarin belongs to Cardiovascular.
Why it happens
Additive anticoagulation, plus argatroban directly prolongs prothrombin time and INR, confounding monitoring.
What to do
When transitioning, follow labeled protocols: use the argatroban-adjusted INR guidance or a chromogenic factor X assay, and stop argatroban only once INR is adequately therapeutic on warfarin. Monitor for bleeding.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
