Summary

Doxepin is sedating and is metabolized partly by CYP2D6 and CYP2C19, and both drugs can cause drowsiness and orthostatic hypotension. At higher doses doxepin may also prolong the QT interval, so additive cardiac effects are possible.

What this interaction means in plain words

Aripiprazole and Doxepin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Aripiprazole belongs to Central Nervous System; Doxepin belongs to Dermatology.

Evidence level: Moderate

Why it happens

Additive sedative, hypotensive and QT-prolonging pharmacodynamic effects.

What to do

Monitor for sedation and orthostasis, and consider an ECG and electrolyte check if doxepin doses are high or other QT-prolonging drugs are present.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
