Summary
Maprotiline can prolong the QT interval, and aripiprazole has a small potential to do so as well. Taken together the effect on repolarization may be additive, with a theoretical increased risk of torsades de pointes. Aripiprazole is among the antipsychotics with the lowest QT liability.
What this interaction means in plain words
Aripiprazole and Maprotiline can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Aripiprazole belongs to Central Nervous System; Maprotiline belongs to Central Nervous System.
Why it happens
Additive pharmacodynamic effect on cardiac repolarization; maprotiline also may raise aripiprazole exposure through CYP2D6 inhibition.
What to do
Obtain a baseline ECG and correct potassium and magnesium before combining. Repeat the ECG after dose changes and reduce or stop one agent if QTc becomes markedly prolonged.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
