Summary

Corticosteroids can increase bleeding risk through vascular fragility and gastric mucosal injury, which may add to the bleeding tendency of low-molecular-weight heparin. Effects on coagulability are inconsistent, and INR is not a useful monitor for bemiparin.

What this interaction means in plain words

Bemiparin and Methylprednisolone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: Low

Why it happens

Additive bleeding risk from corticosteroid-related mucosal and vascular effects together with heparin-mediated antithrombin potentiation.

What to do

Monitor for signs of bleeding, particularly gastrointestinal, and consider gastroprotection during prolonged steroid courses. Use anti-Xa monitoring only where clinically indicated, for example in renal impairment.

What to ask your pharmacist
