Summary
Adding a second ACE inhibitor to captopril-hydrochlorothiazide duplicates therapy and increases the risk of hypotension, hyperkalemia, acute kidney injury and angioedema without added benefit. Dual renin-angiotensin blockade is generally discouraged.
What this interaction means in plain words
Benazepril and Captopril-hydrochlorothiazide can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.
Benazepril belongs to Cardiovascular; Captopril-hydrochlorothiazide belongs to Cardiovascular.
Why it happens
Therapeutic duplication with additive ACE inhibition, causing excessive reduction of angiotensin II and aldosterone.
What to do
Avoid combining two ACE inhibitors and use a single agent. If duplication is found, stop one and check blood pressure, potassium and creatinine.
What to ask your pharmacist
- Does the benefit of taking both outweigh the risk in my case?
- Do I need any blood test, ECG or other check while on both?
- What early warning signs should I watch for, and what should I do if they appear?
