Summary

DPP-4 inhibitors such as saxagliptin have been associated with a higher risk of angioedema when combined with ACE inhibitors. ACE inhibitors may also enhance insulin sensitivity and occasionally cause hypoglycemia in patients on antidiabetic drugs. Both effects are uncommon.

What this interaction means in plain words

Benazepril and Saxagliptin-metformin can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Benazepril belongs to Cardiovascular; Saxagliptin-metformin belongs to Endocrine.

Evidence level: Moderate

Why it happens

Both drug classes reduce degradation of substance P and bradykinin, which may add to angioedema risk; ACE inhibitors may improve insulin sensitivity.

What to do

Counsel patients to report lip, tongue or facial swelling immediately and stop the drugs if it occurs. Monitor glucose for hypoglycemia when starting the ACE inhibitor.
