Summary

Combining a loop diuretic with a thiazide produces sequential nephron blockade and a markedly stronger diuresis. This helps in diuretic resistance but substantially raises the risk of hypokalemia, hyponatremia, hypomagnesemia, dehydration and renal impairment.

What this interaction means in plain words

Bendroflumethiazide and Bumetanide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Synergistic sodium and potassium excretion from blocking reabsorption at both the loop of Henle and the distal tubule.

What to do

Start with low doses and monitor electrolytes, renal function, weight and blood pressure within the first week. Supplement potassium and magnesium or add a potassium-sparing agent as needed.

What to ask your pharmacist
