Summary

Bumetanide-induced hypokalemia and hypomagnesemia increase myocardial sensitivity to digoxin and can precipitate digoxin toxicity and arrhythmias. This is clinically important in heart failure patients who commonly take both.

What this interaction means in plain words

Bumetanide and Digoxin can interact in a way that may be serious. Using them together is sometimes necessary, but it needs a deliberate decision by the prescriber, a plan for monitoring, and clear advice on the warning signs to watch for.

Bumetanide belongs to Cardiovascular; Digoxin belongs to Cardiovascular.

Evidence level: High

Why it happens

Loop diuretic potassium and magnesium depletion enhances digoxin binding to Na+/K+-ATPase and arrhythmogenicity.

What to do

Monitor serum potassium and magnesium regularly and keep them in the normal range with replacement as needed. Check digoxin levels and renal function if toxicity symptoms occur.

What to ask your pharmacist

  • Does the benefit of taking both outweigh the risk in my case?
  • Do I need any blood test, ECG or other check while on both?
  • What early warning signs should I watch for, and what should I do if they appear?
