Summary
Thiazide diuretics can raise blood glucose and reduce insulin secretion, which may weaken the glucose-lowering effect of glipizide. Loss of glycemic control can emerge over weeks, particularly at higher thiazide doses or with potassium depletion.
What this interaction means in plain words
Bendroflumethiazide and Glipizide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Thiazides cause hypokalemia-related impairment of insulin release and may worsen insulin resistance, opposing the sulfonylurea's effect.
What to do
Monitor fasting and post-prandial glucose or HbA1c after starting or adjusting the thiazide, and keep serum potassium normal. Increase the glipizide dose or add other antidiabetic therapy if control worsens.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
