Summary
Bicalutamide is a weak CYP3A4 inhibitor in vitro and has little effect in practice on exposure to most CYP3A4 substrates. Siponimod is cleared mainly by CYP2C9 with a smaller CYP3A4 contribution, so a clinically meaningful rise in siponimod levels is not expected.
What this interaction means in plain words
Bicalutamide and Siponimod have a small, usually unimportant interaction. Most people can take both; the pharmacist may still mention it so that any unexpected effect is recognised.
Bicalutamide belongs to Endocrine; Siponimod belongs to Central Nervous System.
Why it happens
Weak CYP3A4 inhibition by bicalutamide may slightly reduce the minor CYP3A4 clearance pathway of siponimod.
What to do
No dose change is needed; be aware of the interaction if the patient also takes a CYP2C9 inhibitor.
