Summary
Cobicistat is a strong CYP3A4 inhibitor and also inhibits P-gp, which may raise exposure to MMAE from brentuximab vedotin. This increases the risk of neutropenia and peripheral neuropathy.
What this interaction means in plain words
Brentuximab and Cobicistat can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Brentuximab belongs to Oncology; Cobicistat belongs to Miscellaneous.
Why it happens
Inhibition of CYP3A4 and P-gp reduces MMAE metabolism and efflux.
What to do
Monitor blood counts and neuropathy closely, and consider an alternative regimen if toxicity develops. Delay or reduce brentuximab doses for neutropenia per labeling.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
