Summary
Cobicistat strongly inhibits CYP3A4 and P-glycoprotein, markedly increasing colchicine exposure. This can lead to life-threatening colchicine toxicity, and the combination is contraindicated in patients with renal or hepatic impairment.
What this interaction means in plain words
Cobicistat and Colchicine are not normally used together. Taking them at the same time is expected to cause harm or make one of them unsafe, so a different medicine is usually chosen for one of the two.
Cobicistat belongs to Miscellaneous; Colchicine belongs to Genitourinary.
Why it happens
Inhibition of CYP3A4 and P-glycoprotein reduces colchicine metabolism and efflux.
What to do
Do not use together in patients with renal or hepatic impairment. In others, reduce the colchicine dose substantially as per labeling and monitor for toxicity.
