Summary
Bretylium initially releases norepinephrine from sympathetic nerve terminals and then blocks further release. During this phase it can enhance the pressor and arrhythmogenic effects of catecholamines such as dopamine. This can cause marked hypertension, tachycardia or ventricular arrhythmias.
What this interaction means in plain words
Bretylium and Dopamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Why it happens
Bretylium inhibits neuronal catecholamine reuptake and causes transient norepinephrine release, which potentiates the adrenergic effects of dopamine.
What to do
Start dopamine at a low dose, titrate cautiously and monitor blood pressure, heart rate and ECG continuously. Reduce the dopamine dose if there is an exaggerated pressor response.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?

Frequently asked questions
Can I take Bretylium with Dopamine?
MediHelp rates the interaction as moderate. Bretylium and Dopamine can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
How serious is the interaction between Bretylium and Dopamine?
MediHelp rates it moderate, on a four-level scale (minor, moderate, major, contraindicated) based on published labels and references. What to do, in brief: Start dopamine at a low dose, titrate cautiously and monitor blood pressure, heart rate and ECG continuously. Reduce the dopamine dose if there is an exaggerated pressor response.
What other medicines interact with Dopamine?
MediHelp also lists interactions of Dopamine with, for example, Dihydroergotamine intranasal, Dihydroergotamine, Promethazine. The full list is on the Dopamine page.
