Summary

Both loop diuretics and SSRIs such as citalopram can lower serum sodium, so combined use increases the risk of hyponatremia, especially in older adults. Loop-diuretic-induced hypokalemia and hypomagnesemia may also add to citalopram QT risk.

What this interaction means in plain words

Bumetanide and Citalopram can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Bumetanide belongs to Cardiovascular; Citalopram belongs to Central Nervous System.

Evidence level: Moderate

Why it happens

Additive hyponatremia via diuretic sodium loss and SSRI-associated SIADH, plus electrolyte-mediated QT risk.

What to do

Check sodium, potassium and magnesium at baseline and after initiation, particularly in elderly patients. Correct electrolytes and consider ECG monitoring if QT risk factors are present.

What to ask your pharmacist

  • Should I take them at different times of day?
  • Does the dose of either medicine need to change?
  • Is there anything I should avoid, such as alcohol or certain foods, while on both?
