Summary

Using a loop diuretic with a thiazide-type diuretic gives synergistic diuresis through sequential nephron blockade. This increases the risk of hypokalemia, hyponatremia, hypomagnesemia, dehydration and hypotension.

What this interaction means in plain words

Bumetanide and Cyclopenthiazide can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.

Evidence level: High

Why it happens

Additive sodium and potassium excretion through blockade at the loop of Henle and distal convoluted tubule.

What to do

Monitor electrolytes, renal function, weight and blood pressure after starting. Supplement potassium or magnesium as needed and lower doses if volume depletion develops.

What to ask your pharmacist
