Summary
Adding an ACE inhibitor to a loop diuretic, especially in a volume-depleted patient, can cause marked first-dose hypotension and acute deterioration of renal function. The risk is highest at initiation or after a dose increase.
What this interaction means in plain words
Bumetanide and Enalapril can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Enalapril belongs to Cardiovascular.
Why it happens
Diuretic-induced volume depletion activates the renin-angiotensin system, and ACE inhibition removes efferent arteriolar tone, lowering blood pressure and glomerular filtration.
What to do
Consider temporarily reducing or holding the diuretic before starting enalapril and begin with a low dose. Check blood pressure, creatinine and potassium within 1 to 2 weeks.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
