Summary
Adding bumetanide to a thiazide-containing product produces strong additive diuresis, with risk of hypokalemia, hypomagnesemia, hyponatremia, volume depletion and hypotension. Olmesartan may reduce potassium loss but increases renal function risk with volume depletion.
What this interaction means in plain words
Bumetanide and Hydrochlorothiazide-olmesartan can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrochlorothiazide-olmesartan belongs to Cardiovascular.
Why it happens
Sequential nephron blockade by loop and thiazide diuretics produces additive natriuresis and electrolyte loss.
What to do
Monitor electrolytes, creatinine, weight and blood pressure within the first weeks and after dose changes. Replace potassium and magnesium as needed and reduce diuretic doses if volume depletion develops.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
