Summary
Both drugs promote urinary potassium loss, so using them together can lower serum potassium more than either alone. Hydrocortisone has mineralocorticoid activity that adds to the kaliuresis of the loop diuretic. Significant hypokalemia may provoke arrhythmias or muscle weakness, particularly in patients on digoxin or with heart disease.
What this interaction means in plain words
Bumetanide and Hydrocortisone can affect each other. The combination is often manageable, but it may need a change of dose, a change of timing, or extra checks while both are taken.
Bumetanide belongs to Cardiovascular; Hydrocortisone belongs to Endocrine.
Why it happens
Additive potassium wasting from loop diuretic action at the thick ascending limb and mineralocorticoid effect of hydrocortisone in the distal nephron.
What to do
Monitor serum potassium and magnesium periodically, especially with prolonged or high-dose steroid use. Give potassium supplementation if needed.
What to ask your pharmacist
- Should I take them at different times of day?
- Does the dose of either medicine need to change?
- Is there anything I should avoid, such as alcohol or certain foods, while on both?
